Instructions:
Please sign your name under the course number and section that you will be taking evaluations to on the last day of class.
After July 21st, you will find an envelope in your Student Mailbox Folder in the GSLIS Student Lounge. Take the envelope to your final class and distribute the forms to your classmates at the end of the period. The professor should be out of the classroom while evaluations are being filled out. Collect the forms and bring the folder back to the Student Lounge and place it in the top shelf of the black tray on top of the Student Mailbox file cabinet.
